Steps:

- On the thank you page presented to you after submitting this form you will see a link to the upload service. Upload your file is you haven't already done so.
(If you are new to our ProFlo service we will require payment up front for the first few orders you place. Within 24 hours of placing your order you will receive an invoice via email with instructions on how to pay. Once we have established a working relationship we will move our billing cycle to be monthly. )
